    Research Scholar Program Financial Services Billing Campus Mail Campus Safety and Parking Financial Aid Information Technology Internships and Career Services Post Graduate Advising Student Activities and Leadership Programs Student Employment Student Life Saint Anselm College Home Page Current Students Academic Resources Disability Services General Guidelines Main Content General Guidelines Documentation on file for the applicant must contain the following elements It must be current Completed within four years of the student s enrollment at Saint Anselm College Note This requirement does not apply to physical or sensory disabilities of a permanent or unchanging nature A diagnostic statement that clearly states the diagnosed disability or disabilities Specific language regarding evidence of a disability is required terms such as indicative of or suggests a disabilty are not acceptable A description of the functional limitations resulting from the disability or disabilities A description that includes a list of all testing instruments used in the evaluation report and relevant subtest scores used to document the stated disability this requirement does not apply to physical or sensory disabilities of a permanent or unchanging nature A description of the specific accommodations recommended An explanation of why the disability qualifies the applicant for each of the recommended accommodation s in a college environment Typed or printed on official letterhead and signed by an evaluator qualified to make the diagnosis include information about license or certification and area of specialization Please note The diagnosing professional may not be a family member A school plan such as an Individualized Education Program IEP or a 504 Plan is insufficient documentation in and of itself but can be included as part of a more comprehensive evaluative report For detailed documentation guidelines for disabilities please contact Kenneth J Walker Director Academic Resource Center Saint Anselm College 100 Saint Anselm Drive Manchester NH

  • Requesting Accommodations: Initial Request : Saint Anselm College
    three weeks for the evaluation and decision on the documentation submitted When submitting documentation students should include with it the following information their name anticipated year of graduation contact information including phone number home address and e mail address Documentation should be delivered to or mailed to the following address Kenneth J Walker Director Academic Resource Center Saint Anselm College 1607 100 Saint Anselm Drive Manchester NH 03102 Review of Documentation The Director of the Academic Resource Center will review the documentation and the student will be contacted within a few weeks to schedule a meeting Submission of documentation does not guarantee that a request for accommodations will be approved the documentation must meet the guidelines established by the college If documentation is incomplete the student will be contacted and encouraged to meet with the director to review the established guidelines The student may be asked to submit additional documentation II Meeting The student must schedule a meeting with the Director of the Academic Resource Center to discuss the recommended accommodations the resources and services the student should utilize the accommodations the college will provide The student should schedule this meeting during the first two weeks of the semester During this meeting if the documentation is complete the student and the director will complete the accommodations form Please note The requested accommodations are limited to those that are recommended and or for which the student has either received prior approval If there has been a change in the nature of the disability and or the recommended accommodations documentation must be submitted to support the new request The student should then adhere to the guidelines outlined in Part I Medical Documentation III Accommodations Form The Accommodations Form informs the faculty member that the student has a disability and lists the accommodations

  • Arranging Testing Accommodations : Saint Anselm College
    to arrange for their accommodations for each exam during the regular semester Students are contacted at the end of each semester to arrange for final exams I Academic Resource Center Notification Students are expected to notify the Academic Resource Center at least three days prior to their exam for which they will utilize accommodations Students who do not inform the Academic Resource Center at least three days prior may not receive academic accommodations It will depend upon the space availability in the Academic Resource Center and the professor s availability as to whether accommodations can or will be provided Students who arrive at the Academic Resource Center to take an exam who have not notified the Office may be asked to return to the classroom to take the exam with the rest of the class While students may have the right to additional time they also have a responsibility to arrange for their accommodations II Procedure to Notify To notify the Academic Resource Center of an exam students should do one of the following call the Academic Resource Center at 603 641 7181 stop by the Academic Resource Center Cushing 1st floor e mail Caitlin Albright Administrative Assistant calbright anselm edu Students should give the following information when scheduling an exam their name the course name and section the professor s name the date and time of the regular administration of the exam If another class conflicts with the student s ability to use double time on an exam the student must call or stop by the office rather than e mail to discuss scheduling options If a student does not intend to use academic accommodations for an exam he or she does not need to notify the Academic Resource Center III Faculty Notification At least three days prior to

    and Career Services Post Graduate Advising Student Activities and Leadership Programs Student Employment Student Life Saint Anselm College Home Page Current Students Academic Resources Disability Services Renewing Accomodations Request Main Content Renewing Accomodations Request At the Beginning of Each Semester Once the Director of the Academic Resource Center s approval is in place the student must comply with the process outlined below each and every semester Students who are eligible to receive academic accommodations have the right to decline to accept accommodations Meeting Students eligible to receive accommodations and who decide to utilize them must complete the Accommodations Form within the first two weeks of each semester in the Academic Resource Center Cushing 1st Floor It is the student s responsibility to contact the Academic Resource Center 603 641 7181 to schedule a meeting with the director to complete this form Please note The requested accommodations are limited to those for which the student has received prior approval If there has been a change in the nature of the disability and or the recommended accommodations documentation must be submitted to support the new request The student should then adhere to the guidelines outlined for the Initial Request Accommodations Form The Accommodations Form informs the faculty member that the student has a disability and lists the accommodations that will be provided to the student through the Academic Resource Center Depending on the disability the form may also recommend that the student be provided with certain specified accommodations in the classroom The students must bring the following information to complete the Accommodations Form basic identity information such as name e mail address and telephone number their professors first and last names the course names and section numbers Distribution It is the responsibility of the student to distribute the Accommodation Form to each

    Technology Internships and Career Services Post Graduate Advising Student Activities and Leadership Programs Student Employment Student Life Saint Anselm College Home Page Current Students Academic Resources Disability Services Appeals Process Main Content Appeals Process A student may appeal the decision of the Academic Resource Center within ten days of the decision Appeals are directed to the Director of the Academic Resource Center and must be in writing and state the reasons for the appeal and the desired resolution The description must include specific facts to support the appeal A determination of the appeal will be made within ten 10 days of the request The student may appeal the decision of the Director of the Academic Resource Center to the Dean of the College The appeal must be in writing it should state the reasons why the decision of the Director of the Academic Resource Center is being appealed and the desired resolution The appeal should be sent to the Dean of the College within ten 10 days of receiving the decision of the Director of the Academic Resource Center The Dean of the College will make a final determination of the appeal and inform the student of the determination within
