    completed appeals package with supporting documentation to Office of Admissions Bldg 47 RE Admissions Appeal California State University Monterey Bay 100 Campus Center Seaside CA 93955 8001 4 Allow time for processing Appeals decision notification are issued within 6 weeks of receipt of the appeal 5 Receive final decision by mail Students submitting appeals will be notified of the final appeal decision via U S Mail In preparing your appeal please understand the following You may submit one appeal per academic term Appeals must be submitted by mail fax and email appeals will not be accepted Appeals submitted after 15 business days from the admissions decision notification date will not be considered Letters of recommendation will not be considered Reporting errors made on the application are not a basis for the reversal of a decision Appeal letters must be submitted by the applicant Appeal letters written by anyone other than the applicant will not be considered Incomplete packets will not be considered or returned They will be denied and cannot be resubmitted NOTE Students appealing a residency decision should contact the Office of Admissions at 831 582 3738 Students appealing based on a disability should contact Student Disability Resources 831 582 3672 Appeal Types and Supporting documentation Please ensure that your appeal packet contains the following documentation along with your appeal form and letter of appeal request to submit late application Freshmen Official high school transcript and college transcript if applicable issued by the school within the last two weeks Official enrollment verification of courses enrolled or grades if available for fall 2014 and a statement indicating courses that you are enrolled or plan to enroll for spring 2015 Official ACT or SAT Reasoning test scores Transfer Students Official transcripts sent to Cal State Monterey Bay from all colleges universities attended Transcripts must be issued within the last two weeks Official enrollment verification of courses enrolled or grades if available for fall 2014 and a statement indicating courses that you are enrolled or plan to enroll for spring 2015 Official Advanced Placement exam scores and International Baccalaureate transcripts if applicable Military documents if applicable DD214 Joint Services Transcript JST Community College of the Air Force transcript Request to submit late fee Copy of credit card statement canceled check front back or money order receipt that indicates payment to Cal State Monterey Bay was made by the deadline Request to submit late transcripts documents or test scores Transcripts documents or scores and a statement indicating why you were unable to submit them by the deadline Request to apply as a Lower Division Transfer student Official high school transcript issued by the school within the past two weeks Official ACT or SAT Reasoning test scores Official transcripts sent Cal State Monterey Bay from all colleges universities attended Transcripts must be issued within the last two weeks Official enrollment verification of courses enrolled or grades if available for fall and a statement indicating the courses in which you are enrolled or plan to

    Housing Residential Life Open House Counselor Conference Events Near You CSUMB Home Admissions Returning Student Previously attended CSUMB and wish to return Note Students that have filed an official leave of absence with the CSUMB Office of the Registrar are not required to submit a new application for admission Submit a new application through CSU Mentor during the application period csumentor edu 55 00 application fee or CSU fee waiver Must have ALL general education GE coursework requirements completed prior to the term you wish to begin your studies A cumulative GPA of 2 0 or above Submit new transcripts from all institutions previously attended except for CSUMB Be in good standing at the last school attended All requirements must be met by the semester before attending CSUMB Spring 2016 applicants transcripts are due by October 15 2015 Fall 2016 applicants transcripts are due by March 15 2016 Academically Disqualified DQ from CSUMB Wait a minimum of one year two semesters from the term you were disqualified before attempting to appeal Additional semesters may be necessary Take a minimum of 12 transferable units Additional units may be necessary Pass ALL transferable units with a grade of C or better Cumulative GPA of 2 4 or greater for all transferable units taken after disqualification from CSUMB A cumulative GPA of 2 0 or above for ALL units including CSUMB units Must have ALL general education GE requirements met Submit a new application through CSU Mentor during the application period csumentor edu 55 00 application fee or CSU fee waiver Submit an appeal letter to the Director of Admissions Submit a completed Disqualification Appeal Form Submit new transcripts from all institutions previously attended except for CSUMB All requirements must be met by the semester before attending CSUMB Be in good standing at

    Events Near You Financial Aid Student Housing Residential Life Open House Counselor Conference Events Near You CSUMB Home Admissions Veterans Applying CSUMB is a full participant in the CSU Troops to College Program Veterans and active duty military or spouses receive admissions priority however all regular upper division transfer requirements must be met in order to be admitted Applications are submitted on csumentor edu as an upper division transfer student If you are unsure that you meet upper division transfer requirements contact our office MILITARY CREDIT College credit may be given for military training or job duties as listed on a veteran student s official military transcript Joint Services Transcript JST or CCAF Air U Specialized military schools such as the Defense Language Institute also have separate transcripts to be submitted The Office of Admissions will determine what credit to assign based on the American Council on Education ACE credit recommendations along with our own evaluation How much credit is assigned will vary by student Military credit is often vocational in nature and will not replace academic credit Veterans will need to attend a 2 year or 4 year college prior to CSUMB to meet all transfer admission requirements VETERAN
