    Expired identification DMV photo receipts and Sacramento State One Cards will not be accepted per Department of Justice requirements Examples of acceptable forms of identification are a California drivers license a California identification card an out of state drivers license a current passport an Resident Alien registration card or immigration card green card a military identification card or a Mexican Consulate Identification Card Matricula Consular de Alta Seguridad Applicants must also bring three 3 copies of your BCII 8016 Request For Live Scan Service Form with the ORI Code Type of Application Job Title etc Agency Address Set Contributing Agency and the applicant s personal information filled in prior to your appointment time This form must be obtained from the person requiring your fingerprint submission Applicants must bring a check with them to their Livescan appointment If you do not have the ability to write a check please call 916 278 2788 and speak with the Live Scan Operator to arrange to pay your fees at Lassen Hall We cannot accept cash or credit card payments at the time of your appointment How much will my application cost An application has two fees 1 A rolling fee and 2 A pass through fee s The rolling fee is set by the Sacramento State Police Department and is 10 00 per application The pass through fee s are set by the Department of Justice and the Federal Bureau of Investigations Additionally other various fees may apply if your application requires additional checks through other various State and Federal databases Depending on your application the fees can cost anywhere from 0 00 to as high as 76 00 The cost of your application is unknown until the AORI code listed on your application is entered into the Live Scan device at the time of your appointment Currently the Department of Justice has set their pass through fee at 32 00 and the Federal Bureau of Investigations has set their pass through fee at 19 00 Please be advised that the DOJ and the FBI reserve their right to change their fees at any time without notice All fees must be paid for prior to submitting your fingerprint transmission Please make the appropriate arrangements to pay your fees How do I fill out the BCII 8016 Request For Live Scan Service Form The information that you are responsible for on the BCII 8016 Request For Live Scan Service Form is the area field that starts with Name of Applicant For a detailed explanation of how to fill out your form please download this BCII 8016 8016A Instruction Form in pdf format Please read over this information carefully My fingerprints were rejected what do I do If your fingerprints are rejected by the Department of Justice you will need to bring with you the rejection letter from the Department of Justice in order to have your fingerprints resubmitted If your fingerprints were submitted at the Sacramento State Police Department the 10 00 rolling fee
