    Program Description Application EOP Brochure Parents Guardians and Counselors Parents Guardians and Counselors Application Resources Sacramento State Resources Forms Equity Programs CAMP SASEEP FSMP SSWD PARC GSP Website Parent Family Website MLK Website DRC Website EOP 45th Anniversary Strategic Planning and Assessment Vice President For Student Affairs Office Transfer EOP Application Process EOP at Sacramento State is different than EOPS at the Community college The EOPS grant advising requirements and other services received at the Community College will be different than the services we offer Our emphasis is on thorough timely advising and career guidance Due to financial limitations not all students will receive the EOP grant If admitted to EOP the following services are offered Academic counseling Career counseling EOP computer lab EOP Ambassadors peer advising EOP Grant to eligible students and if funding permits Financial Aid advising Learning communities Personal counseling Speakers lectures workshops Referrals to Faculty Student Mentor Program FSMP Referrals to other departments or agencies Please Note EOP stands for Educational Opportunity Program and EOPS stands for Extended Opportunity Programs and Services In order to complete your EOP application it is critical that you meet the deadlines listed below Please Note EOP receives close to 4 000 applications we only accept 200 Transfer applicants Failure to adhere to a deadline will result in your EOP application being withdrawn from consideration 1 Sacramento State Admissions filing period October 1st November 30th You must apply for admission to Sacramento State before applying to EOP Complete the EOP section in the admissions application section 11 submit your online Sacramento State Admissions Application at www csumentor edu Once you submit your Admissions Application and it is determined that you are eligible you will automatically receive access to the online EOP Application Note A separate application is required for EOP 2

  • Freshmen Agreement Plan
    to support your educational goals we are here to help you academically succeed We will provide the services below from enrollment at Sacramento State through your senior year Please review the statements below describing the requirements and services offered GENERAL PROVISIONS I agree to participate in the EOP Summer Bridge program This is a summer program held at Sacramento State beginning the first week in July through August Dates to be determined I agree to maintain full time enrollment 12 units per semester in order to be eligible for the EOP Grant as stated in the EOP Grant Criteria and my full time financial aid awards If my GPA should fall below a 2 00 or if am placed on Academic Probation I agree to meet with my EOP counselor for advising academic planning and or counseling I agree to seek tutorial assistance in EOP as directed by my instructor counselor or teaching assistant I agree to have my course schedule revised if necessary to be in accordance with EOP program requirements As a First Year Student Fall Semester and Spring Semester I agree to participate in the EOP Learning Communities Program in the fall semester Placement in a learning community will be contingent upon my EPT ELM test scores my class level standing and my EOP admission status I agree to meet with my EOP counselor at least once per semester or as requested by my counselor I agree to participate in the spring semester Transitions Program which requires enrollment in Ethnic Studies 98 As a part of this program I agree to meet with my EOP Counselor at least twice in the spring semester or as instructed by EOP As a Continuing Student Second Year through Graduation If my GPA should fall below a 2 00 or if
