    Catalog HIGH SCHOOL GPA if available HIGH SCHOOL RANK ASSESSMENT DATE Date student took the entry level assessment test s DATE PRINTED Date the form was printed CATALOG YEAR Catalog that contains degree plan that student must complete ECU MAJOR ADVISOR HIGHEST COMPOSITE ACT SCORE Center of Form AREA English Math Reading and Science are the areas tested by the ACT and by entry level assessment tests HIGHEST ACT SCORES Highest ACT sub score is the highest score that the student has attained in the corresponding area of the ACT If the student has taken the test more than once the scores may be from different ACT tests and may not equal the Highest Composite ACT Score listed at the top of the form COURSE PLACEMENT English Math Reading and Science Students with scores above 19 in any area or students who score above the cut score on the placement test s will be UNRESTRICTED Students who have a skills deficiency score below 19 on the subject area and score below the cut score will be RESTRICTED and the course that they are required to take will be listed Curricular deficiencies student did not take required high curriculum are noted by SEE CONTRACT the student contract will be in the student s advisement folder Refer to the contract for required courses and time limits For more information see Policy of Remediation of High School Curricular Deficiencies in the current catalog As noted by the required courses for curricular and skills deficiencies in English Math and Science must be completed within the student s first 24 hours attempted For more information on curricular and skills deficiencies see the Policy of Remediation of High School Curricular Deficiencies in the current catalog form Computerized Placement Test InformationFor entry level assessment students take the
