    contained in the student s education records FERPA authorizes disclosure of what is considered directory information for all student records without consent if no modifications to access are made Directory Information Directory information includes name degree or certificate program class participation in activities dates of attendances degrees or awards received and the most recent previous educational agency or institution attended An annual notification of FERPA policies shall be made available to students and shall include the types of education records and information designated by Erikson Institute as directory information Directory information may be released to the following person s or for the following reason s without consent School officials who have a legitimate educational interest if the official needs to review an education record in order to fulfill his or her professional responsibility Other schools to which a student is transferring Specified officials for audit or evaluation purposes Appropriate parties in connection with financial aid to a student Organizations conducting certain studies for or on behalf of the school Accrediting organizations To comply with a judicial order or lawfully issued subpoena To military recruiters under the Solomon Act Appropriate officials in cases of health and safety emergencies and State and local authorities within a juvenile justice system pursuant to specific State law Limiting Access to Directory Information Any student who does not desire inclusion of some or all of directory information in the Institute directories or other dissemination of the information by the Institute should notify the Registration and Student Records Office in writing no later than October 1 of each academic year concerned Request to Block Release of Directory Information PDF 1pg Students may not prohibit the verification of attendance or of the awarding or lack of awarding of a degree or certificate nor may they prohibit Erikson school officials from gaining access to information used in a professional capacity Allowing Access to Educational Records Educational records excluding directory information are only accessible to third parties with the written permission of the student with the exception of Erikson school officials Also in cases of death the executor of the estate is allowed full access to student records with appropriate documentation Exemptions to Educational Records The following documents are not educational records for the purposes of FERPA Records kept in the possession of the maker that are used only as a personal memory aid and not accessible to any other person Employment records related solely to a student s capacity as an employee of the Institute except where a current student is employed based on student status e g work study graduate assistantship etc Records maintained for compliance with local law enforcement Medical and mental health records made maintained or used by professionals in connection with the treatment of a student that are available only to persons providing treatment and Records with information about an individual created after she he is no longer a student at the Institute The right to inspect and review the student s education records Students

    plans applying for aid student services writing program computers technology housing transportation health insurance registration and records academic policies enrollment and degree verifications name and address changes transcript request forms immunization u pass library online offerings online advantages how it works recommended skills tech requirements self assessment continuing education offerings webinars webinar archives quick credits credit types faq customized training family child care modules achieving biliteracy research community initiatives assessment children at risk family support infants toddlers schools teaching technology young children supporting erikson contribute to erikson ways to give areas of support give online corporate foundation gifts consumer information The Illinois Department of Public Health requires that all students attending a post secondary institution show proof of the following immunizations before registering for classes Two doses of a live measles vaccine administered at least one month apart and after first birthday One dose of a live rubella vaccine administered after first birthday One dose of a live mumps vaccine administered after first birthday One diphtheria tetanus Td booster within ten years of enrollment Proof of immunity or a diagnosis of measles mumps and rubella is also acceptable documentation Acceptable documentation must contain the month day and year of the vaccine proof of immunity or the day the disease was conferred accompanied by a licensed health care provider s signature In accordance with the above stated requirement all degree and certificate students enrolled in an on campus program at Erikson and registered for 3 or more credits are required to submit a completed Immunization Record Form Forms are included in the admission materials sent to all admitted certificate and degree program students and are also available from the Registration and Student Records Office When completed the forms should be returned to the Assistant Director Registration and Student Records The following
