    the Florida Relay at 711 General Information Health Plans Prescription Drugs Dental Plans Vision Plans Reimbursement Accounts Limited Purpose Medical Reimbursement Cost Estimator Medical Reimbursement Cost Estimator Dependent Care Reimbursement Cost Estimator Florida Gulf Coast University offers benefits to Faculty Administrative Professional A P and Support Personnel SP employees These benefits may include paid annual and sick leave holidays state and university sponsored insurance programs and retirement Due to the temporary classification of OPS and Adjunct Faculty positions these employees are not eligible to participate in university sponsored benefit programs Benefits are a valuable component of your employee compensation and the University is extremely proud to present our benefits package to you You have an impressive selection of benefits to choose from so review the following information and carefully consider your personal benefit needs New Faculty Administrative Professional A P and Support Personnel SP employees have only the FIRST 60 CALENDAR DAYS OF EMPLOYMENT to enroll in the insurance plans of their choice After the first 60 days employees may only enroll during the annual Open Enrollment period which takes place each year in the fall for coverage effective January 1 st of the following year You may be allowed to make certain changes to your coverage if you experience a Qualifying Status Change QSC event e g marriage divorce birth or adoption of a child etc You must notify the People First Service Center within 60 days of the QSC Employees are eligible for health insurance coverage on the first day of the month following their date of hire However the effective date of coverage will depend on the date the enrollment is completed and the required premiums are paid The effective dates of coverage are always the first of a month and no coverage will be updated with

    information including OPS eligible benefits 5 Are there any State sponsored benefits that I am NOT eligible for OPS employees will not be able to enroll in a Medical Reimbursement or Limited Purpose Medical Reimbursement Account or Optional Life Insurance 6 Who will pay for the cost of my benefits The employee is responsible for their portion of the premium see above and the hiring department is responsible for paying the employer portion of the premium This applies to the Health Insurance only 7 When will coverage start if I am eligible to enroll during Open Enrollment Coverage will begin on January 1 2014 If an eligible employee enrolls during Open Enrollment payroll deductions will begin in December 8 What are the measurement and stability periods The new hire measurement period is 12 consecutive months beginning the first day of the first month following an OPS employee s hire date for which the average hours worked weekly will be measured to determine eligibility for coverage The open enrollment measurement period is also 12 consecutive months from October 1 through the following September 30 for which the average hours worked weekly will be measured to determine eligibility for coverage for the subsequent plan year January 1 through December 31 The stability period is the 12 month period following a measurement period during which eligible employees who enroll in coverage must be covered Following the OE measurement period the stability period is January 1 through December 31 9 Once enrolled in insurance how long will I get to keep my coverage Health insurance coverage continues for 12 months your stability period as long as you continue working for the state even if your hours drop below an average of 30 Eligibility to continue coverage for the subsequent plan year will be determined annually by measuring your hours worked during the open enrollment measurement period If you are determined to be eligible coverage continues without interruption and you will be guaranteed another 12 months of health insurance coverage If you are determined to be ineligible health insurance will end the last day of the twelfth month of coverage All other plans will end at the same time as health insurance regardless of the number of months you are enrolled If you terminate employment you are covered through the last day of the month following termination provided premiums are paid in full You can continue health dental and vision coverage through COBRA for up to 18 months if you were enrolled in those plans at the time of termination 10 What happens if I have coverage but I m determined to be ineligible during the next open enrollment measurement period All coverage will terminate at the end of your 12 month health insurance stability period however you can continue health dental and vision coverage through COBRA for up to 18 months if you were enrolled in those plans at the time of your ineligibility Eligibility for the subsequent plan year will be measured during

    000 group term life and accidental death and dismemberment policy The university pays 100 of the cost of the coverage for full time employees and contributes toward coverage for part time employees on a pro rated basis according to their percentage of employment State Optional Term Life Insurance Non Pre tax Employees may enhance the basic life coverage by selecting amounts from one to seven times base annual salary up to a maximum of 1 000 000 Although this is an employee pay all benefit the premiums are extremely competitive Based on the level of coverage selected medical underwriting may be required Supplemental Insurance Pre tax Supplemental insurances include several dental insurance options Humana United Healthcare Assurant CIGNA and Ameritas one vision plan through Humana two cancer intensive care insurance options AFLAC or Colonial Life two supplemental health options Alta or New Era accident disability insurance from Colonial Life These are all pre tax benefits available to state employees at group rates Retirement Plans Faculty and A P employees may choose between the following retirement plans Optional Retirement Plan ORP Available to all Faculty and A P employees Each pay period the University contributes a percentage based on an employee s gross earnings to a state approved investment provider The employee is required to contribute a mandatory 3 of their gross earnings Employees can elect to contribute an additional percentage of earnings on a voluntary basis subject to IRS regulations ORP providers are outlined in the Employee Benefits Guide Florida Retirement System FRS The FRS provides two retirement options the Pension Plan and the Investment Plan The FRS Pension Plan is a defined benefit plan Contributions are made to the Pension Plan by the University based on the employee s FRS membership class Employees are required to make a 3 contribution The Pension Plan requires 8 years of creditable service to vest and receive a retirement benefit This system is combined with Social Security to assist you by providing an income for your later years or for an unforeseen disability Once vested you may retire at age 65 and receive full benefits or at an earlier age with reduced benefits You may retire at any age after 30 years of service with no reduction in benefit The monthly benefit payment you will receive when you retire depends on your years of creditable service and the average income of the eight highest years fiscal year during your covered employment The FRS Investment Plan is a defined contribution plan The University contributions are based on your salary and your FRS membership class Regular Class Special Risk Class etc Employees are required to make a 3 contribution The Investment Plan requires 1 year of creditable service to vest i e own the assets in your Investment Plan account The Investment Plan directs contributions to individual member accounts and you allocate your contributions and account balance among various investment funds Your Investment Plan retirement benefit is based on the value of your account Unlike
