    leave has been credited to the employee at the time of enrollment Part time employees employed at less than 1 00 FTE may participate in the Pool on a pro rata basis At the time of enrollment each employee contributes eight 8 hours of sick leave to the Pool When the Pool balance drops below eighty 80 hours Pool members are required to contribute an additional eight 8 hours of personal sick leave All members will be notified of the Pool s depletion and given an opportunity to inform the Pool Administrator in writing if they intend to discontinue membership prior to the deducting of personal sick leave credits to replenish the Pool balance Any participating employee may cancel his her membership at any time by notifying the Sick Leave Pool Administrator in writing Any sick leave contributed to the Pool is forfeited upon cancellation of membership retirement or termination No employee will be unreasonably denied membership in the Sick Leave Pool if eligibility requirements are met ENROLLMENT PERIODS The Sick Leave Pool Committee will designate a minimum of one 1 open enrollment period consisting of at least thirty 30 calendar days during each twelve 12 month period All eligible employees who wish to voluntarily join and participate in the Sick Leave Pool must apply for membership during one of the open enrollment periods Eligible employees who complete their first year of service at other than during an open enrollment period will have thirty 30 calendar days to apply for membership in the Sick Leave Pool An employee may transfer his her Sick Leave Pool membership from another state agency or SUS university during the first thirty 30 calendar days of employment at the University The employee is responsible for determining the point in time he she may become eligible for membership in the Sick Leave Pool ADMINISTRATOR AND SICK LEAVE POOL COMMITTEE The President will appoint six 6 employees who participate in the Pool to serve as the Sick Leave Pool Committee The Assistant Vice President Director of Human Resources has been designated as the Sick Leave Pool Administrator The Sick Leave Pool Committee consists of two employees respectively from the Faculty A P and Support Personnel employee groups Committee members for each employee group are appointed to serve alternating two 2 year terms ending December 31st UTILIZATION OF THE SICK LEAVE POOL All Pool hours will be disbursed by action of the Sick Leave Pool Committee and membership in the Sick Leave Pool does not guarantee hours may be withdrawn from the Pool Sick Leave hours from the Sick Leave Pool will be granted only for an employee s serious health condition which requires hospitalization or extended medical care as the result of any injury or illness or as a result of major medical or health problems Personal illness includes disabilities which are the result of or are contributed to by pregnancy related illness and the recovery therefrom Participating employees who have depleted or will shortly deplete all

    2 Tele 239 590 1400 Fax 239 590 1011 If you have a hearing or speech impairment call Florida Gulf Coast University using the Florida Relay at 711 Special Pay Plan Special Pay Plan Overview W hat is the Special Pay Plan The Special Pay Plan was a mandatory 403 b retirement plan to faculty and staff employees funded through accrued annual sick and compensatory leave cash outs at the time of separation and or DROP participation or when transferring out of an annual leave accruing position Effective Date of the Plan The Special Pay Plan was implemented effective July 1 2004 and was repealed on July 1 2014 Current Status of the Plan The Special Pay was frozen July 1 2014 which means that the plan remains intact but no new contributions are permitted If you already have funds invested in the plan you will be eligible to receive it just as you would have before the freeze Withdrawals Withdrawals from the plan may be made at the following times Separation termination of employment Retirement After age 70 or retirement if later when the IRS requires that minimum distributions be made to the participant each year Participant s total

    THE JOB INJURIES OR ILLNESSES TO REPORT A CLAIM CALL OUR WORKERS COMPENSATION CARRIER AmeriSys TOLL FREE 1 800 455 2079 Reporting an on the Job Injury or Illness to AmeriSys In the event of a job related injury or illness AmeriSys staff will assist the injured employee in receiving prompt quality and medically necessary care as well as maintaining the employee in an active work status either in modified or transitional duty What happens in the first 2 4 hours following a work related accident will determine overall success in both human medical outcomes and financial claims cost terms AmeriSys provides 24 7 365 claim reporting through our Triage Unit The role of the triage nurse is a key component of AmeriSys unique and proactive claim management program At the time the claim is reported the triage nurse will orient the employee to the workers compensation system demonstrate concern regarding the injured employee s well being determine the level of medical care required and make the necessary medical referral After the employee is directed for medical care the claim will be assigned to the nurse case manager and adjuster team who will manage the claim to conclusion Once you have filed your claim with AmeriSys you must then report the injury to the University Environmental Health Safety Department at 239 590 1126 What are the employer s responsibilities and what can be expected following an on the job injury or illness In the case of a medical emergency call 911 and then immediately contact AmeriSys at 800 455 2079 to report the claim If the injury is not an emergency the supervisor should immediately report the claim to AmeriSys Whenever possible please have the injured employee present with the supervisor when the claim is reported The triage nurse will
