    Only Spring 2016 Only The lender will retain a loan origination fee from the PLUS Loan For example if you request 1000 the lender will retain 4 272 or 42 and the remaining 958 will be disbursed to you If my PLUS loan is denied my child wishes to receive additional Unsubsidized Loan funds in his or her name Yes I authorize JWCC s Financial Aid Office to initiate a credit check which is required for a Parent PLUS Loan Yes If a credit balance occurs after my PLUS Loan is disbursed please distribute the remaining balance to My child student Myself parent Are you currently in default on a federal education loan or do you owe a refund on a federal student grant YES NO Borrower s Rights and Responsibilities Carefully read and review your borrower s rights and responsibilities listed below We encourage you to print out a copy for your records Check marking each box indicates you have read and accepted each statement Financial aid including loans will be used first to pay tuition and fees at JWCC if a credit balance occurs it will be used for educational expenses Your child may not receive financial aid from two schools at the same time Your child must be enrolled and actively attending at least 6 hours towards a program leading to a degree or an eligible certificate at JWCC If your child s enrollment status changes and or he or she receives additional aid after the loan has been processed the Financial Aid Office may be required to reduce or terminate your loan Classes that begin late in a semester may delay loan disbursements Federal PLUS loan checks are delivered in two equal disbursements per semester You have the right to reduce or cancel your loan prior
