    Schedules Registration Your Healthy Life Contacts Sitemap Personnel Action Request PAR Quality Assurance At some future date the institutions will implement the Electronic Personnel Action Form EPAF available in the Banner HR human resources information system HRIS However the time required to develop troubleshoot and implement EPAFs for particular personnel actions means that we may have to rely on the presentPersonnel Action Request PAR form for an indefinite additional time As the volume of PARs reaching Human Resource Services Budget and Payroll is exceptional and since a fair percentage of these PARs contains errors that require their return to the client who tendered them we seek to improve the quality and processing of the PAR The information below will hopefully assist clients in preparing PARs that will be free of errors and that will be processed without delay The three administrative offices Human Resource Services Budget and Payroll appreciate the opportunity to serve clients through processing personnel actions with PARs However problems may arise simply from the volume of PARs that are coming through these offices The volume of PARs may prevent one on one interactions with clients who prepare PARs Hopefully a quality assurance page such as this one may reduce the number of problems with PARs Proper and correct content on PARs is very important PARs that are not complete and correct must be sent back to the client for revision and re submittal This can introduce delays in getting people paid in getting employment set properly in getting persons enrolled in benefits and in a number of other areas The solution is better quality assurance in PAR content and processing The guidelines below will prove helpful The PAR is located on the Human Resource Services website at http www marshall edu human resources forms in Adobe PDF and Word formats The Adobe version can be completed on line and printed Please use an ink color other than black for signatures Signatures in a color other than black helps those processing the PAR to distinguish the original form from copies Effective dates must reflect the actual beginning and or ending dates of employment A termination date may be corrected through a subsequent PAR if the originally stated termination date is not correct All temporary employees Extra Help Casual and or Part Time Faculty must have a beginning AND ending date Names should be exactly as they appear on the person s Social Security Card not nicknames The Social Security number must be accurate and exactly the same as it appears on the individual s Social Security card In addition to the employee s Social Security number please enter his her Banner ID number Date of birth gender race and citizenship are required for any new hire The Wage Rate line needs to be completed with the exception of PARs for overtime or a change of address On Type of Action the Employment box is checked ONLY for the initial hire not for overtime change of rate transfer or

    a stated owner and one or more associates Therefore a process can always be linked to a person Ownership however does not denote personal ownership All processes are common property owned jointly by ourselves and our customers It is expected however that the necessary expertise to provide or facilitate a process will be provided by a particular individual This owner guides and improves the process A process does not necessarily require all of a person s time to perform Some processes can be performed by a person or less and a person can be involved in several processes NEAR TERM LONG TERM VIEW Planning for human resource processes takes both a near term and a long term view Human resource processes are visualized which can adequately support the institution now and in three to five years An attitude of flexibility and innovation is adopted in order to support present needs and respond to new requirements or new opportunities in the future TECHNOLOGY The incorporation of technology into the provision of human resource processes is anticipated and planned for to the extent that technology is available and cost effective Technology as applied to the human resource processes is a process in its own right and requires the assignment of time and resources in order to aid the other human resource processes MINIMIZING THE IMPACT OF TIME AND PLACE The contemporary human resource function must be mobile must interact with people where they work and must make as many services as possible available without regard to time of day Whenever possible customers must not be obliged to come the human resources office for assistance The human resource processes are managed in the expectation that human resource staff will get out and network with their customers and stakeholders Given the geographical separation among the parts of the University the processes will be organized and will apply technology in such a way that the importance of time and place for the provision of services is minimized The goal is processes that are seamless with regard to the location of supported employees BANNER HR The University is utilizes SCT Corporation s Banner HR database product as its human resources information system HRIS Banner HR offers rich opportunities to enhance the efficiency and effectiveness of human resource processes Workflow distribution which brings personnel actions down to the user s desktop is encouraged The tracking and support of human resource processes including employment leave benefits and classification have become easier and more efficient with the adoption of Banner HR WEB BASED INTRANET BASED TECHNOLOGY Web based and other technologically supported processes significantly enhance the provision of the human resource processes To the extent possible such resources are utilized in order to increase the ease and convenience of customer contact and the speed of transactions Due provision is made to assist employees who may not have computer access This web page reflects the importance of web functionality in the provision of human resource services PROACTIVITY A proactive mind

    do not accrue leave The policy and the leave request form are available at the links below Completed leave request forms are to be maintained by a designated individual in each college or department Presently that should be the same individual who prepares or coordinates the monthly leave usage reports Later when leave usage is handled through Banner HR time reporting the forms should be maintained on some reasonable college or department basis Colleges and departments may request approval of alternate leave forms that satisfy the requirements set forth in the policy see fourth paragraph in the policy Such requests are to be made to the Director Human Resource Services Colleges and departments must obtain approval for alternate leave request forms by the end of January 2002 or use the standard leave request form effective February 1 2002 Anytime thereafter however requests for alternate leave request forms may be submitted for approval The leave request forms are the official record of leave approval for that particular college or department A key concept in the policy and form is that the amount of leave taken in a particular instance must exactly match that documented on the leave request form or information on the actual amount of leave taken must be entered on the form The form provides an area titled Reconciliation for adjusting the amount of leave For example if a leave accruing full time employee requests two days of annual leave 15 0 hours of annual leave must be reported for that individual on either the present leave usage report or through some future method of time reporting However if the employee returns to work after just one day of annual leave the leave request form must be updated to reflect that only 7 5 hours of annual leave were
