    but if this fails they will copy the exam from your laptop print and return it to you No attempt to fix your computer will occur during an exam BROWN ENVELOPE You must complete all the information requested on the brown envelope including your exam number The brown envelope will hold all your materials to be turned in at the end of exam If you are taking a Exam 4 please write a big E4 on the exam envelope DO NOT PUT YOUR NAME ON ANY EXAM MATERIALS ANSWER SHEETS Scantrons If answer sheets are used put your exam number not your name on the answer sheet provided Remember to put the version A or B if applicable of the exam on the answer sheet Also please bring 2 pencils for your exam Pencils will not be supplied for students NEVER PUT YOUR NAME ON ANY EXAM MATERIALS GENERAL INSTRUCTIONS Students are to write the examination in ballpoint pen no felt tip pens Do not write in the margins of the exam booklet After the exam packets are distributed and prior to the beginning of the exam the proctor will instruct you to remove everything from the brown envelope Put the exam face down on the desk Complete the information on the brown envelope If you are using bluebooks put your exam number on the cover of every bluebook and also number each blue book If you are using Exam 4 log on to the program up to the opening screen No typing is allowed before the exam begins Complete scantron when necessary for exam Remember your exam number goes where it says name Never write on scrap paper before exam begins When the proctor instructs you to turn over your exam the proctor will allow you to count all pages and write your exam number on each page The proctor will then read the Anonymous Grading Policy Rule to you You will then be allowed to silently read the Professor s instructions When everyone is finished reading the instructions the proctor will instruct you to begin your exam DO NOT BEGIN THE EXAM UNTIL INSTRUCTED TO DO SO NO OTHER WRITING OR TYPING IS PERMITTED PRIOR TO THE COMMENCEMENT OF THE EXAM Students are reminded to keep their eyes on their own examination It is the responsibility of all proctors to safeguard against cheating Any requests by a proctor pertaining to the exam seat assignments scrap unnecessary noise snacks etc should be taken seriously by the student During the course of the examination the proctor is the person in authority and should be treated as such Passing blue books books papers snacks etc between one another is strictly forbidden If you require additional bluebooks or scrap just silently signal to the Proctor or you may come up to the desk The Proctor will put the current time on the blackboard every half hour or so Ten minutes before the exam ends the Proctor will announce that you have ten

    when a written notice or form is submitted to the Dean of Students The date the form is received by the Dean of Students or the postmark on a mailed envelope whichever is earlier will be the official date of withdrawal Likewise the date a student withdraws online using MyPace Portal will be on the official date of withdrawal Non attendance of classes informing the instructor of withdrawal or stopping payment on a check does not constitute official withdrawal and does not relieve the student of their financial obligation or entitle the student to a refund It is important to note that all charges and cancellations are based upon tuition commitments for the full semester Tuition Cancellation Schedule Time of Withdrawal Note Weeks are counted as seven calendar days starting with the first day of the term including holidays THE APPLICATION AND INSTITUTION FEES ARE NON REFUNDABLE Fifteen or Sixteen Week Semesters most Fall and Spring courses Cancellation Prior to the opening day of the 15 16 week semester 100 During 1st week of the semester 80 During 2nd week of the semester 70 During 3rd week of the semester 25 During 4th week of the semester 20 During and after the 5th week of the semester 0 Eight Week Semester Summer Session Cancellation Prior to the opening day of the eight week semester 100 During 1st week of the semester 75 During 2nd week of the semester 50 During 3rd week of the semester 20 During and after the 4th week of the semester 0 For additional weeks please contact the Office of Student Assistance Student Accounts Registrar Calendar weeks will be used in determining elapsed weeks from the opening day of the semester For first year students the first day of Orientation Week instruction is the opening date of the semester The effective date of withdrawal for computing any cancellation due to the student is the day when the written application is granted by the Dean of Students Application reactivation and installment fees deposits and the other miscellaneous fees listed on this page are not refundable Students who have elected to pay on the installment plan are responsible for completing all payments if a balance exists after tuition cancellation Military Service Any student required to discontinue attending classes because of induction into a United States military service may be given a complete refund of all tuition and fees except the application and general institutional fees if orders to report for active duty are received within the first two weeks of class Thereafter students may choose either a pro rated refund or an application of full credit of tuition and fees excluding the application and general institutional fees to a possible future enrollment Serious Illness If a student withdraws from all his her classes during the first half of the semester due to serious illness a pro rated cancellation of tuition and special course fees will be permitted The policy applies to the student s personal illness only and
