    Release and Authorization coming 2016 University Activity Informed Consent Liability Waiver coming 2016 Please click on PDF for the items above to open the form Clearly print on all forms and fax mail or drop off in person by the deadline Please do NOT mail Please contact alberto gutierrez sjsu edu with any questions The Educational Opportunity Program strives to help students be successful It is of utmost importance to start your undergraduate studies on a firm foundation EOP Summer Bridge offers that firm foundation for new freshmen students 2016 Program Dates TBA Selection Participation in this academic opportunity is considered a privilege Selection is based on admission to SJSU EOP NextSteps completion academic need ELM EPT scores educational and personal background Adherence to the program is central to participant success Non attendance at Summer Bridge may affect your admission status to SJSU and EOP you may file an EOP Status Appeal if you have extenuating reasons for non attendance Academics Join a select group of 60 EOP freshmen participating in Summer Bridge Get a jump start on your college education and experience life at SJSU Participate in 5 weeks of intensive academic studies designed to fulfill remedial requirements and to develop needed writing math and study skills On Campus Living EOP Summer Bridge is a residential live on campus program You will be required to live on campus for 5 weeks to experience the full impact of the program FREE There is no fee to attend other than personal travel expenses to and from the campus Food on campus housing and academic materials will be provided by EOP EOP Summer Bridge will also meet your CSU Early Start requirement s Orientation Summer Bridge students will participate in SJSU Orientation Session Participants will enroll in predetermined courses remedial and general

    been mandated to attend EOP Tutoring The more availability and specific instructions you provide the better chances of a tutor match Tutor appointments will be set up for the entire semester and can take 3 5 days to process Requesting Tutoring 1 Sign into GradesFirst 2 Click Request Tutor Appt link for class you want to request tutoring in 3 Include ALL your available days times between M Th 9am 7pm Tutoring is only available beginning on the hour in one hour increments Ex a request for 9am 10am can be matched a request for 9 30am 10 30am can NOT be matched 4 Include special instructions tutor preference work schedule etc 5 All tutoring appointments are set for the entire semester if you are not required to attend tutoring be sure to indicate how many tutoring sessions you would like to have 6 Indicate group or individual tutoring 7 Select location to EOP Self Service 8 Click Send to submit Tutor Request Please be aware that almost all of the tutoring is in group sessions Tutoring appointments fill up quickly so you may need to provide additional availability if you did not provide sufficient enough information in your request Good Example Mondays Wednesdays 11am 2pm Tuesdays Thursdays 3pm 6pm Bad Example Monday 9am 10am Tutoring Check in out 1 Check in at the EOP Front Desk 2 Swipe in with your Student ID card through GradesFirst 3 Your tutor will come out to greet you in the lobby 4 Swipe out after your tutoring session Appointment Cancellations 1 Sign into GradesFirst 2 Click on the appointment via the Calendar tab 3 Click Cancel Appointment 4 Follow prompts Cancel VS No Show You must cancel a tutoring session 24 hours before your appointment without being counted as a no show

    The university s priority filing date is typically between October and November The filing date for students entering SJSU in fall is December 15th Students must turn in 2 letters of recommendation and their FAFSA CA Dream Act to EOP by February 1st Prospective students must also file for FAFSA CA Dream Act this is a requirement in order to verify financial eligibility to EOP In order to be given priority EOP processing please submit your FAFSA by no later than February 1 for a fall term Students that apply for FAFSA after 2 1 through 3 2 closing priority deadline for all students may not receive priority consideration Q Do EOP applicants automatically qualify for an application fee waiver A No Applicants must demonstrate a financial need to be considered for the fee waiver Determination of eligibility will be made when applying to the university online at CSUMentor Only California residents are eligible for application fee waivers If you file the admission application via CSUMentor the fee waiver request is included in the online system as part of the application process You will be notified at the time you apply online if you qualify for the fee waiver In situations where the online application is unable to determine your eligibility for a fee waiver you will need to submit the Request for Application Fee Waiver directly to the Admission office The Admission Office will inform you if you do not qualify for the fee waiver All fee waiver questions should be directed to the Admissions Office Q How do I check the status of my EOP acceptance A Due to the high volume of applications and recommendations submitted EOP Can not check the individual status to see what has been submitted on your behalf Please follow up with your recommender and or CSU Mentor to track what recommendations have been submitted for you Acceptance into SJSU does not guarantee acceptance into the Educational Opportunity Program EOP acceptance criteria does differ from SJSU admission criteria and admission into EOP is competitive Q How do I check the status of my admission to SJSU and documents needed A For questions regarding admissions you will need to contact the Admissions Office directly at 408 283 7500 You may also check your Admission To Do list online at MySJSU Q How do I know if I am even eligible to apply for EOP A If you meet the criteria below you should apply toe EOP EOP is for undergradute students applying for the first time to SJSU If you are transferring from another CSU or a CCC you must have been an EOP EOPS student at your prior campus in order to be eligible at SJSU Please request the prior school to send verification of EOP status directly to EOP at SJSU by the EOP application deadlines Prospective students must meet income guidelines as found online at CSUMentor Prospective students must be the first in their family to attend college Prospective students must

    Aid and Scholarship Office for more details Click on the following questions below and it will direct you towards the answer Is applying to EOP the same as filing the FAFSA How do I qualify for the EOP Grant Do I need to reapply for financial aid every year Does EOP at SJSU offer book vouchers or daycare assistance Q Is applying to EOP the same as filing the FAFSA A No The two filing process is different Students filing the FAFSA must submit any required documents to the Financial Aid Office in order to verify EOP eligibility and be considered for any financial aid grants work study and loans Q How do I qualify for the EOP grant A The EOP grant is awarded based on full time enrollment 12 units each semester at SJSU EFC expected family contribution cannot exceed 1 500 total amount of funding available and the date that the all of student s required forms have been submitted to the Financial Aid Office The EOP grant is a maximum of 1 500 per academic year Amount awarded is based on funding availability If you have not received the EOP grant please contact the Financial Aid Office directly at 408 273 7500 for more information If you do not receive the EOP grant you may be eligible to receive other types of aid grants scholarships and loans Actual aid awarded to students is determined by your need demonstrated on the FAFSA application you filled out prior to the deadline Q Do I need to reapply for financial aid every year A Yes The FAFSA application period opens on January 1st of every year and the priority deadline closes on March 2nd the same year The earlier you submit your FAFSA form and the sooner you submit

    bridge program for new students A Yes Incoming freshman that do not meet the regular admission criteria and are admitted through a special admit process are required to attend an intensive Summer Bridge program You can find information about it on our Summer Bridge page Q Why would a student who is currently earning good grades benefit from EOP A All students need supportive communities to be successful Very few people succeed on their own EOP has a dedicated group of trained professionals that can help foresee and address challenges that students may encounter at the college level Q Can I still receive EOP support services if I don t receive the EOP Grant A Yes EOP will provide with you academic support services even if you do not receive the grant Q Does EOP offer workshops A Yes At EOP we host workshops throughout the semester that are specifically geared towards EOP students Please check your requirements to see how many workshops you have to attend Q Can I be removed from EOP A EOP does not typically remove students from the program if the student is meeting all EOP responsibilities All students in the program sign a detailed Student Agreement that pledges their active participation in program However if a student is not holding up their end of the student agreement they can be removed from EOP Q What types of services does EOP provide to students A EOP provides information on and explanations of academic policies procedures and regulations General Education and university graduation requirements a Four Semester Educational Plan for freshmen assistance with class scheduling career counseling personal counseling and advocacy on the behalf of students It also offers tutoring and mentoring services through students Q Does EOP have a website A You re on it
