    Enrollments Dependent Plan FAQs Dental and Vision Insurance Options Pharmacy About Us Pharmacy Services General Overview Pharmacy Services Filling a Prescription Obtaining a Refill Payment Options Special Services Emergency Contraception Physical Therapy About Us Physical Therapy How We Treat and Provide Care Our Staff Physical Therapy MORE Physical Therapy Inc Travel About Us Travel Health Our Services Travel Health Services Eligibility and Cost Travel Health Services Scheduling an Appointment Travel Health Services Vaden s Travel Health Online Module Travel Health Services Staff Travel Health Resources You are here Home Health Insurance Cardinal Care Student Health Plan Medical Mental Health and Dental Benefits Cardinal Care is structured to complement the care that is available to students through Vaden Health Center Medical Benefits Cardinal Care s medical benefits are administered by Health Net of California An Exclusive Provider Organization EPO plan Cardinal Care lets you decide where to get medical care The cost at the time of care is dependent on where you receive your treatment You can choose from one of the following options Tier 1 Medical Care Stanford University Medical Center or Menlo Medical Clinic Tier 1 coverage allows students to be seen at Stanford University Medical Center or Menlo Medical Clinic with a referral from a Vaden Health Center clinician For office visits and most outpatient services you pay only a 20 to 30 co payment and Cardinal Care covers the balance A co payment is the fee you pay each time you receive a service For inpatient care at Stanford hospital coverage is at 100 for the 2014 2015 plan year and at 100 after a 500 copay per admission starting with the 2015 2016 plan year Tier 2 Medical Care Any Health Net Preferred Provider including Stanford University Medical Center or Menlo Medical Clinic Tier 2 coverage allows students to receive care from any Health Net preferred provider This flexibility can come in especially handy when travelling since Health Net s network is local national and international For the 2014 2015 plan year when Tier 2 is in effect Cardinal Care pays 80 of eligible expenses after you meet a 200 annual deductible For the 2015 2016 plan year when Tier 2 is in effect Cardinal Care pays 70 of eligible expenses after you meet a 500 annual deductible A deductible is the amount you pay each year before Cardinal Care begins reimbursing services Tier 2 allows you to use Stanford University Medical Center or Menlo Medical Clinic without a referral from your Vaden Health Center clinician For Tier 2 benefits outside of California you must utilize a FIRST HEALTH network provider Services rendered out of the country are reimbursed at the Tier 2 benefit level Be sure to follow the directions on your individualized ID card when accessing health care off campus For more information on medical benefits call 1 800 250 5226 Health Net of California or visit www healthnet com cardinalcare and look at My Health Net Plan under the Cardinal Care tab Mental

    your university bill as well as other important details by quarter based on when your enrollment begins at Stanford There is no option to change the coverage period or to prorate the premium Students may contact Vaden s health insurance office at 650 723 2135 or by email healthinsurance stanford edu for questions regarding waiving insurance coverage 2015 2016 Academic Year The annual cost for Cardinal Care for the 2015 2016 academic year will be 4 680 The annual plan which runs from September 1 through August 31 includes coverage in Summer quarter whether you are registered that quarter or not The 2015 2016 Cardinal Care Cost and Coverage Table includes graduate subsidy provides information about the charges you will see on your university bill as well as other important details by quarter based on when your enrollment begins at Stanford There is no option to change the coverage period or to prorate the premium Students may contact Vaden s health insurance office at 650 723 2135 or by email healthinsurance stanford edu for questions regarding waiving insurance coverage Cardinal Care Subsidy for Graduate Students Frequently Asked Questions Stanford subsidizes the Cardinal Care premium for graduate students who are employed in research or teaching assistantships and for students receiving an equivalent level of fellowship support Details regarding eligibility and amount can be found on the Stanford Financial Aid website 1 How much is the subsidy Certain students will qualify for the full benefit of the Cardinal Care subsidy while others will qualify for the partial benefit The FULL benefit consists of a payment of 50 percent of the Cardinal Care premium The PARTIAL benefit consists of a payment of 25 percent of the Cardinal Care premium For the purposes of calculating the amount of the subsidy the Cardinal Care premium is defined as the cost of the 12 month coverage divided by three academic quarters 2 Who qualifies to receive this benefit The following enrolled graduate students who do not waive Cardinal Care and do not have an outside fellowship source paying the full cost of insurance will receive the FULL subsidy those with a Research or Teaching Assistantship appointment of 25 or more those with a fellowship paying a non tuition stipend at or above the minimum salary for a 25 assistantship CA or RA The following enrolled graduate students who do not waive Cardinal Care and do not have an outside fellowship source paying the full cost of insurance will receive the PARTIAL subsidy those with a Research or Teaching Assistantship appointment of less than 25 those with a fellowship paying a non tuition stipend at or above the minimum salary for a 10 assistantship CA or RA Students not meeting the above criteria are not eligible for a Cardinal Care subsidy 3 Will eligibility be determined on an annual or quarterly basis Eligibility will be determined on a quarterly basis Students will be eligible based on the financial assistance they receive in a quarter Likewise the subsidy

    and international students so please read the text carefully Enrollment Considerations for ALL Students Cardinal Care is an ANNUAL enrollment health insurance plan The annual fee is billed over three quarters for four quarters of coverage For the 2014 2015 academic year the annual cost of Cardinal care will be 4 296 Students who enter the university in Autumn will be charged 1 432 in Autumn Winter and Spring quarters and will not be billed in Summer quarter For the 2015 2016 academic year the annual cost of Cardinal care will be 4 680 Students who enter the university in Autumn will be charged 1 560 in Autumn Winter and Spring quarters and will not be billed in Summer quarter Students enrolled in Cardinal Care are covered during leaves of absence breaks immediately following graduation and quarters in which they are not registered for classes including Summer quarter Charges for Cardinal Care are prorated for students whose first registered quarter at Stanford is Winter Spring or Summer If a student begins in Winter coverage is for eight 8 months January 1 through August 31 and charges will occur over the remaining two quarters of the academic year Winter and Spring If a student begins in Spring coverage is for five 5 months April 1 through August 31 and the entire charge will occur in Spring quarter If a student s first quarter as a registered student is Summer Cardinal Care can be purchased for the Summer quarter only In this case coverage is for three 3 months June 1 through August 31 and the charge will occur in Summer quarter Remember Since the plan is an annual one a student may make the decision to waive coverage or to remain in Cardinal Care only once each academic year Cardinal Care Coverage When Leaving the University Cardinal Care is a full year plan that is if you enter Stanford in the Autumn quarter and do not take steps to opt out of Cardinal Care by the applicable deadline you ll have coverage for the entire year and be charged accordingly Similarly if you enter Stanford in other quarters and do not take steps to opt out of Cardinal Care by the applicable deadline you ll have coverage for the remainder of the academic year and be charged accordingly The one exception to the annual coverage requirement applies to students who graduate at the end of Autumn or Winter quarter Students who plan to permanently end their relationship with the University through conferral of a terminal degree at the end of Autumn or Winter quarter may request to be taken off the plan at the end of the corresponding Cardinal Care coverage period December 31 for Autumn graduates and April 30 for Winter graduates To initiate a request you must complete the Petition for Early Cancellation of Cardinal Care Health Insurance form and submit it to Vaden s Insurance and Referral Office by the applicable deadline December 15 for Autumn graduates and
