    from you is not provided promptly Be very careful when completing loan applications to provide all information requested All changes you make on the application form should be initialed Consult a Student Loan Administration counselor if your financial situation changes after you apply for assistance The counselor can determine if because of new circumstances an adjustment can be made to your financial eligibility Report in writing the receipt of any financial assistance not offered through SLA including receipt of departmental or outside agency fellowships Outside scholarships count toward meeting your financial need If additional University assistance or outside assistance puts you in an over funded status your loan and or work study assistance may be reduced and or you may be billed for amounts already received Be sure to tell your lender s about any change to your name or address You are responsible for repaying all loans whether or not you receive bills requesting repayment During your attendance at the University you must update any name telephone number or address change at my uchicago edu You may also be required to forward documentation of a name change to our office You may appeal the decision made on your student loan application if you believe it does not accurately reflect your current financial situation or if your financial situation has changed substantially since you first submitted your application Please start by completing the Student Budget Appeal form NOTE As part of the release of financial information to a third party you must authorize Student Loan Administration to discuss your financial assistance information with a third party e g your spouse parent or landlord otherwise we cannot release information specific to your financial assistance without your written permission You may complete the authorization on your application If you did not complete

    read the repayment schedule carefully and be sure you understand it before you sign it Your lenders will also provide you with a monthly billing statement Grace Periods Under most loan programs you will be allowed a grace period between the time your registration falls below half time and the time you have to begin repayment This is to allow you time to get established in a new job and a new living situation Interest on all subsidized Federal Stafford Loans will not begin to accrue until the end of the grace period however you are responsible for interest on all unsubsidized Federal Stafford Loan from the date of disbursement Please refer to your promissory note for length of grace period Deferments If you qualify for deferments you must contact the lender for a deferment form You must see that the deferment form is completed and certified by the appropriate school or agency official and returned to the lender Always follow up with your lenders to verify that deferments have been received and processed You may be required to make loan payments until your lender receives the form Loan Forgiveness under the Stafford Loan The Loan Forgiveness Program cancels your obligation to repay all or part of your outstanding principal and interest on Stafford loans or consolidation loans you used to repay those loans You may qualify if you Teach elementary school or secondary school and are a highly qualified teacher Teach elementary school and can demonstrate your knowledge and teaching skills in reading writing math and other areas of the elementary school curriculum Teach secondary school in a subject area relevant to your academic major You are a full time elementary or secondary special education teacher or a full time secondary math or science teacher NOTE You must complete

    who receive Title IV Federal student aid loans work study and grants to make satisfactory academic progress SAP toward completion of their degree in order to continue receiving student aid Title IV Federal Aid That Falls Within These Guidelines Federal Work Study Federal Perkins Loan Federal Stafford Loans subsidized and unsubsidized Federal Graduate PLUS Loan Federal TEACH Grant Three components to maintaining satisfactory academic progress includes Maintaining an acceptable cumulative grade point average of at least 2 0 or the minimum required by the division school for graduation Completing a minimum of at least 66 of all attempted cumulative units whether units count towards degree or not Successfully completing degree required coursework within 150 of the normal time frame allotted by their division school Students Who Fail To Earn At Least One Passing Grade In A Given Quarter If you fail to earn at least one passing grade for a quarter the University must treat you as an unofficial withdrawal and return 50 of the Title IV funds disbursed to you Grades must be submitted and posted to the University s Registrar system within 30 days from the last day of the quarter If you receive Title IV Federal aid
