    and order a drug or device to an APRN or a PA Increasing the number of APRNs and PAs to whom a physician may delegate prescriptive authority from four to seven FTEs In practice sites serving medically underserved populations and in hospitals there is no limit on the number of APRNs to whom a physician can delegate just as in current law Ensuring patient safety by improving communication and coordination between the Medical Board Board of Nursing and Physician Assistant Board regarding physicians APRNs and PAs who have entered into prescriptive authority agreements Allowing physicians to delegate prescriptive authority for Schedule II controlled substances to APRNs and PAs in hospitals and hospice settings The Texas Medical Board is prohibited from adding requirements to the prescriptive authority agreement QA process that are not in statute and Removing geographic distance limitations on any practice and requiring no minimum amount of time that a physician must be on site Facility based practices hospitals and long term care providers will continue as they are with some changes being discussed All other practices will be combined into a single category with uniform requirements for physician involvement with APRNs Overall coordination should be improved between the Texas Medical Board Texas Board of Nursing and the Texas Board of Physician Assistants A companion bill HB 1055 from Representative Lois Kolkhorst of Brenham is in the House These bills empower a more collaborative delegated practice that allows members of the healthcare team to practice to their level of education and training This proposed change is fully consistent with one of the four key messages recommended by The Future of Nursing Commission which was sponsored by the Robert Wood Johnson Foundation and the Institute of Medicine IOM Nurses should practice to the full extent of their education and training Indeed the Coalition for Nurses in Advanced Practice Texas Nurses Association Texas Nurse Practitioners and Texas Association of Nurse Anesthetists as well as the Texas Academy of Physician Assistants commend the proposal I note with pride that our 2010 Doctor of Nursing Practice graduate Christy Blanco from El Paso is on the TNP Executive Committee and has been very involved in the public testimony process Unlike other bills in other sessions SB 406 is considered an agreed to bill and has received a statement of support from the Texas Medical Association TMA Texas patients will have better access to more efficient high quality team based medical care which will ensure patients get the right care at the right time in the right place and from the right healthcare provider said the TMA s president Dr Michael E Speer of Houston in a news release With the imminent increase in the number of uninsured who will soon have access to insurance and projected physician shortages an important consequence will be a dramatically increased demand for primary care providers There will be a greater need for nurses in Texas hospitals and clinics Clearly APRNs will take a larger role in primary care

    debated political topic in recent years as all branches of government struggle with how best to distribute limited medical resources Chronic illnesses that cost our country through significant morbidity and mortality have been the focus Diabetes is one such chronic illness pervasive through all segments of society increasing in numbers and often resulting in poor outcomes despite advances in medical treatment In 2007 alone diabetes cost the U S about 174 billion with almost 58 billion lost due to disability decreased productivity and early mortality Medical expenses for people with diabetes are two times higher than for those without diabetes There are a number of different terms that refer to diabetes which can lead to confusion Sugar diabetes type 1 or type 2 impaired fasting glucose and glucose intolerance may all be used by either patients or healthcare providers to describe diabetes But what exactly is diabetes Diabetes is a group of diseases marked by abnormally high blood glucose levels which are the result of aberrations in insulin production function or both Elevated blood glucose levels in turn cause alterations in the functioning of most other organs including the heart kidneys or eyes causing diabetes related morbidity Healthcare providers strive to control blood glucose because the better the control the fewer the complications and the fewer the complications the better the patient outcomes Finding just the right balance between diet activity and medications is the key to reducing these complications Why then with the advances in our understanding of cause and effect do so many people still experience complications Effective control of high blood glucose is complicated Not only must medications be prescribed and taken appropriately and effectively but diabetic patients must make choices every day about what to eat when to eat how much activity is enough how much activity is too much or what to do when sick Basic nutrition classes in public school may give us an idea of what we should eat to be healthy but this basic knowledge does not prepare us for what to do when facing a lifelong potentially debilitating illness that is directly related to our diet and the activity we do or don t do every day Education to help people gain the knowledge needed to manage diabetes is known as Diabetes Self Management Education or DSME In DSME participants gain practical information while practicing skills to manage diabetes Curricula in these programs can vary but most address healthy eating and activity levels taking medications developing positive coping mechanisms problem solving and monitoring Centers for Medicare Services CMS and independent insurance providers recognize programs accredited by either the American Diabetes Association ADA or the American Association of Diabetes Educators AADE and they generally cover the cost of diabetes education delivered in these settings DSME is considered a standard of care by the ADA and all those diagnosed with diabetes should receive DSME UT Health Services UTHS our primary care nurse managed clinic located in the Texas Medical Center offers an accredited

    and identifying possible courses of action An ombudsman can help resolve the problem in most cases however complaints involving serious abuse or neglect are referred to the appropriate agency In all situations confidentiality is maintained and no information is released without permission of the resident or legal guardian Provide Information and Assistance An ombudsman is a good source of information about selecting a long term care facility eligibility criteria and other services for the elderly If you would like a packet of information about nursing homes and or assisted living facilities in Harris County please feel free to contact us and we can mail a packet of information to you Advocate for System and Legislative Changes State and regional ombudsman programs work cooperatively with other advocacy organizations to recommend legislation and regulatory changes that affect older Texans Staff Ombudsmen routinely serve on boards and committees of other organizations and actively advocate for policies to promote quality of care What Kind of Training and Support do Volunteers Receive All volunteers receive training prior to becoming certified Trainings are held on Saturdays Training I lasts 4 hours and Training II which is held a month later lasts 8 hours In addition to the trainings continuing education sessions are held four times a year These educational sessions provide volunteers with current information about issues in long term care Staff ombudsmen are always available to answer any questions volunteers may have as well as provide them with support and guidance in their role as advocates for long term care residents What Skills are Needed to Become an Ombudsman Becoming an ombudsman is easy If you Are a good listener Have good negotiating skills Are good at problem solving Are an advocate You would make a good Ombudsman How to Become an Ombudsman To find

    in the field with seniors This has been the most enjoyable project I ve ever done in earning my masters degree said Zemanek an occupational therapist enrolled at Texas Woman s University Working with the other disciplines makes it fun because you see how they tackle the project from their point of view They chose the Nintendo game after researching what the seniors wanted and assessing where they could have an impact We created avatars for all the seniors which makes it personalized It gives them something to look forward to Ransing said Their attention span is less so they need something short in length We tried some of the other Wii sports but the existing games took too long for them so we settled on bowling Team AARP Astute Alliance of Respectable Practitioners chose a different route At the Third Ward Multi Service Center they concentrated on giving seniors important information about their healthcare in a way they could easily digest We re passing on the same information that they have heard before but in a different package one that s focused on them said Stephen Powell a student at the UTHealth School of Nursing We re teaching the basic things for health In the Third Ward cardiovascular disease is the biggest killer so it s critical for us to talk about this topic On a recent Thursday Donald Lefeber a student at the College of Acupuncture Oriental Medicine talked to the seniors about the importance of whole body health Brandon Bailey a pharmacy student at the University of Houston educated them about medications One resident told Bailey that if she misses a dose of her cholesterol medication she will double up on it when she takes the next dose and asked him if that was OK He explained

    faculty member tackled barricades mud pits and fire jumping and also wore Viking horns on their heads I think these guys were trying to show that there are many ways to have fun and be healthy even in school said Renae Schumann Ph D R N associate professor of clinical nursing in the Department of Acute and Continuing Care It s not so great to graduate in nursing then not be physically fit enough to do the job Waves of more than 500 participants attempted to complete a 3 1 mile course by crawling under climbing hand over hand jumping over swimming through and generally overcoming an intimidating series of obstacles Each warrior had the opportunity to raise funds for St Jude Children s Research Hospital or another charity Warrior Dash does not contribute any of their revenue to the Memphis based hospital rather all of the money raised for St Jude and other non profits comes directly from the participants Some of the money raised goes to Wounded Warriors so it holds a lot of meaning said Schumann Also we are always promoting fitness and exercise in nursing school so we are showing how it can be done The School of Nursing team was assembled Sort of last minute said Schumann by Team Captain Nathan Aaron Cox and included fellow BSN students Gabriel Blog Daniel Kerr Candice Schrodetzki Traci Van Matre and Amy Vidalin Obstacles on the extreme 5K run included The Trenches crawl Barricade Breakdown hurdles Capsized Catamaran swim Cliffhanger rappel Hard Rain deluge trudge Road Rage scrap yard stampede Mud Mound slippery trek Great Warrior Wall tall barricade Cargo Climb nets Warrior Roast fire leap Muddy Mayhem under barbed wire to the finish Schumann said that some members of this team plan to run the 2013 Warrior
