    of that employment shall be compensated at full salary An employee also has the right to additional time off as provided for in the Family Medical Leave Act of 1993 Personal Convenience Leave A Leaves of absence without pay for reasons of personal convenience will not be approved in situations where a replacement is needed Conversely if a replacement is not necessary the leave may be approved by the Director of Human Resources B Prior to approval of such leaves vacation leave must be exhausted to the extent that only five working days remain in the vacation leave bank of the employee if the leave is for more than 50 percent or only ten days remain in the accrued leave bank if the leave is for 50 percent or less C Exceptions to the above policy may be approved only in situations where critical emergency situations occur approval will then be at the discretion of the Director of Human Resources or the supervising Vice President D Insurance benefits may be continued if the employee pays the employee and state portion of the cost during the leave Contact Benefits for arrangements to continue insurance Retirement credit is not available during a personal convenience leave Pregnancy Leave Pregnancy is considered a medical condition and therefore any request for leave related to pregnancy is considered a request for medical leave There is no separate leave policy for pregnancy Also See Medical Leave and FMLA Top Contents Benefits While On Leave Holiday Vacation Sick Pay While on leave of absence without pay employees will not receive holiday pay nor accrue additional vacation or sick leave benefits Insurance Coverage Employees may continue employee group insurance by making direct payments to CMS if applicable To determine if state supported basic insurance will continue while on a specific leave contact Benefits 298 1853 Retirement Benefits Employees who are granted a qualifying leave of absence without pay and who return to their position at the end of the leave for eight months or the period of the leave whichever is less may file an election to pay employee contributions during the leave to receive service credit for retirement Ability to contribute depends on the reason for the leave without pay Employees may continue to receive disability death and survivors coverage for up to 120 days during the leave without pay even though they do not elect to pay contributions during the leave Check with Benefits 298 1853 or SURS 1 800 ASK SURS for details Top Contents Library Privileges The University Library is a center for academic study and individual research as well as a place for personal reading and enjoyment It is available to all Civil Service employees of the University seven 7 days a week The facility is located on Western Avenue and has a large parking lot convenient for those using automobile transportation The resources of the library include approximately one million cataloged volumes and subscriptions to 3500 periodicals and 24 newspapers Information about specific

    regulations Employees who terminate employment on the last scheduled working day prior to a holiday do not receive holiday pay as they are no longer employees in a status position on the holiday Top Contents Identification Cards Photo ID cards are available at no cost at the Validine Office of Student Residential Programs in Seal Hall Information is available in Human Resources 298 1971 Top Contents Insurance BOT II C 7 The State of Illinois provides each full time status employee with comprehensive health vision dental and life insurance The employee cost for the insurance is based on the plan chosen and the annual salary rate of the employee Employees working less than full time but half time or more hired on or after January 1 1980 must pay a pro rata share of the basic cost for their coverage and may choose life only health dental vision only or the full insurance plan Each eligible employee must complete enrollment forms in Human Resources within the first ten calendar days of employment Employees may choose health vision dental and life insurance for eligible dependents with no evidence of insurability if purchased within the first ten calendar days of employment Otherwise a completed questionnaire on each dependent s health must be submitted for approval by the insurance company to apply for coverage The dependent must be approved for coverage to be added Changes of insurance coverage are generally allowed during Benefit Choice period once a plan year or when the employee has a qualifying change in family circumstance Each full time employee has basic term life insurance in the amount of their annual salary rate Optional term life insurance is available for the employee spouse and or eligible children Additional term life insurance is available for the employee spouse and or children Two accidental death and dismemberment plans are available Information and enrollment forms for the insurance programs are available from Human Resources 298 1853 Top Contents Interviewing for Positions Current employees may interview for other positions on campus during their regular working hours without loss of pay with supervisory approval Employees interviewing during working hours without supervisory approval of release time must use approved vacation leave Employees must request approval in advance from their supervisor Supervisors are expected to approve requests within a reasonable amount of time to allow the employee to be considered for the vacancy Top Contents Job Descriptions For each Civil Service position at WIU a current job description must be on file with the Classification Division of Human Resources as required by the State Universities Civil Service System In accordance with the State Universities Civil Service System if there is no job description on file for the specific position or if the existing description has not been reviewed and updated during the past three years it will be necessary to do so before any Request for Personnel form will be acted on by Human Resources To avoid any delay when filling a vacancy an updated description

    a pro rata basis of one working day for each month of full time service and are cumulative with no limit Employees are expected to personally notify their supervisor as soon as possible of illness requiring absence from work and to report the absence on their timecard Human Resources reserves the right to request a doctor s release the doctor to be approved by Human Resources before an employee can return from an extended period of sick leave or in cases of possible excessive use or abuse For bi weekly paid employees the amount of sick leave accrued is reported each pay period on the time card For CSE or semi monthly paid employees a printout is sent to each department monthly Sick leave may be used for a personal illness or b illness or injury of a parent spouse or child up to 12 days per calendar year Effective March 29 1984 an employee whose employment or reemployment by the Board of Trustees of Western Illinois University commences within two years after termination of his or her employment by an employer covered by the State Universities Civil Service System shall if separated in good standing be allowed to transfer his or her accrued sick leave to the location of employment or reemployment within the Board of Trustees of Western Illinois University system provided that such employee has not received a lump sum payment for accrued sick leave If such employee has received a lump sum payment for accrued sick leave only accrued sick leave which was earned before January 1 1984 may be transferred credit for accrued sick leave which was earned after December 31 1983 shall be governed by Section II C 7 d of BOT Regulations Lump sum payments for sick leave upon termination are made in accordance with BOT Regulation II C 7 d 4 If an employee is entitled to and applies for a retirement annuity from the State Universities Retirement System which is to begin within 60 days after termination of employment covered by this System or one of the other systems subject to the Illinois Reciprocity Law the employee will receive additional service credit for unused and unpaid sick leave Service credit for unused and unpaid sick leave will be granted in accordance with the following schedule as of January 1979 Number of Full Work Days Additional Service Credit 20 59 1 4 year 60 119 1 2 year 120 179 3 4 year 180 259 1 year 260 299 1 1 4 year 300 1 1 2 year This formula is subject to change by the State Universities Retirement System For up to date and more extensive information contact Human Resources 309 298 1853 Top Contents Status Changes Address Marital Status Dependents It is the employee s responsibility to keep records current when personal status changes occur such as change of address telephone number marital status and or number of dependents Employees can complete a change of status form and new W
